Consumer Assistance Council

The Consumer Assistance Council offers a Consumer Complaint Form provided by the Massachusetts Attorney General. You may download one here and complete it at home. NOTE: This is a special kind of PDF form which allows you to use your desktop computer to fill it out.

When done, please choose “Save As” with your own name in the file name, then print and mail it to:

Please do not mail it to our office as it will delay the processing of your complaint.
