The Consumer Assistance Council offers a Consumer Complaint Form provided by the Massachusetts Attorney General. You may download one here and complete it at home. It is a special kind of PDF form which allows you to use your desktop computer to fill it out.  When done, please “SaveAs” with your own name in the file name, then email it to us.

Otherwise, you can print it and  do it by hand and mail to us at the address below.
